Duties
- Conduct comprehensive assessments of patients’ physical function, mobility, and pain levels in home settings
- Develop and implement individualized care plans aligned with clinical standards and patient goals
- Perform therapeutic interventions such as therapeutic exercises, manual therapy, trigger point therapy, and modalities to facilitate recovery
- Collaborate with healthcare professionals including physicians, nurses, and occupational therapists to coordinate patient care and discharge planning
- Maintain accurate and timely clinical documentation in electronic health records (EHR) systems following documentation standards
- Educate patients and their families on treatment techniques, safety precautions, and home exercise programs to promote ongoing rehabilitation
- Monitor patient progress regularly and adjust treatment plans as necessary to optimize outcomes
Experience
- Prior experience practicing in a physical therapy role within home health or outpatient rehabilitation settings is essential
- Demonstrated knowledge of physiology, anatomy, orthopedics, neurology, and geriatrics to tailor effective treatment strategies
- Experience working with geriatric patients, pediatrics, or neurologic conditions is highly desirable
- Familiarity with inpatient hospital care, outpatient clinics, or outpatient rehabilitation centers enhances your ability to deliver comprehensive care
- Proficiency in medical terminology and clinical documentation standards ensures clear communication and compliance
- Hands-on experience managing electronic health records (EHR) or electronic medical records (EMR) systems is required
- Strong client communication skills are vital for engaging patients effectively in their care plans
