What are the responsibilities and job description for the Bilingual FAYS Family Support Facilitator position at Project Unity?
Job Overview
Performs and ensures the completion of a variety of professional duties to accomplish the goals of the Family and Youth Success program (FAYS). Family Support Facilitators use Solutions Focused Brief Therapy (SFBT), Parenting Wisely, and Coping CAT to coach parents and their children in learning life skills to improve their relationships and decrease conflict through support and behavior management. Must be willing to travel up to 400 miles a week throughout the counties that the FAYS program serves. This position will primarily serve Falls, Limestone, Leon and/or Milam Counties; however, the FAYS program serves many counties including Brazos, Burleson, Grimes, Madison, Leon, Washington, Robertson, Milam, Limestone, and Falls. Spanish fluency is required.
Responsibilities
- Serves approximately 65-70 FAYS participants annually/25 FAYS participants monthly on caseload
- Conducts intake/assessments on each FAYS participant
- Develops service plans with FAYS participants based on assessment
- Provides coaching and/or parent education for youth and/or their parents based on their needs in a variety of environments, including participants’ homes as needed
- Provides resource coordination, service coordination and basic needs assistance for participants as needed
- Ensures all required paperwork (pre and post surveys, enrollment and discharge forms, sign in sheets, etc.) is completed fully and signed by participants
- Ensures all data is entered daily, fully, and correctly in Intake1; Project Unity's household management system
- Assists supervisor in outreach and recruitment activities in assigned counties
- Attends biweekly team meetings and individual appointments with the supervisor monthly
- Performs other duties as assigned
Experience
- An Associate degree or higher in a health and human service field OR
- Two (2) years of direct service experience in a health and human services field
- Must be able to complete self-directed online training
- Must possess good time management and organizational skills
- Must have experience working with a diverse group of people from all walks of life
- Must be comfortable providing community and home-based services to families
- Spanish bilingual required

Pay: $42,000.00 - $45,000.00 per year
