Essential Duties And Responsibilities
The essential duties of the Case Manager include, but are not limited to:
- Conduct pre-intake interviews to determine appropriateness for housing at St. Nicholas House.
- Complete intakes, assessments, and psychosocial evaluations in accordance with agency standards.
- Efficiently utilize the AWARDS digital case management system to input and update all client information and documentation.
- Provide case management services, supporting the creation and implementation of individualized Service Plans with both short- and long-term goals.
- Participate in clinical team meetings to discuss high-risk clients and update tracking spreadsheets.
- Provide short-term supportive counseling and advocacy on behalf of tenants.
- Organize and facilitate support groups, tenant activities, and events both on- and off-site.
- Maintain communication with internal and external service providers on behalf of clients.
- Identify service needs and make referrals for medical, mental health, psychiatric, substance use, and other supportive services.
- Escort clients to essential appointments and meetings when necessary to ensure proper follow-through.
- Perform other duties as assigned by the Clinical Director or Program Director.
- Bachelor’s degree in social work, psychology, or another human services field from an accredited college or university.
- Minimum of 1–2 years of experience working with homeless individuals, individuals with mental illness, or other special needs populations preferred.
- Strong teamwork, organizational, interpersonal, communication, and computer skills; bilingual abilities are a plus.
- Ability to work cooperatively with peers and staff to effectively and efficiently meet client needs.
- Ability to work independently and adjust one’s schedule to accommodate client needs.
- Excellent oral, written, and listening communication skills.
- Ability to effectively interface with clients and staff at all levels.
- Proficiency in Microsoft Office Suite, especially Outlook, Word, and Excel.
