SALARY: $55,000-$65,000

Project PLASE (People Lacking Ample Shelter and Employment) was founded to remedy the lack of housing and resources available to Baltimore’s homeless citizens. Our mission is to address homelessness by providing temporary and permanent housing and supportive services to homeless adults and families, particularly the most vulnerable and undeserved (i.e. people with mental illness, HIV/AIDS, addictions, ex-offenders, etc.) and by treating the whole person and empowering each individual to function at the highest possible level. We currently operate two temporary housing facilities and are in the process of converting an old elementary school in Irvington, Baltimore into 90 permanent and temporary affordable housing units.

Job Summary: The Housing and Hope for Veteran Case Manager will provide intensive case management services and support to Veterans and their families and who may have complex needs such as serious and persistent mental illness, history of substance abuse, history of incarceration, history of chronic homelessness, or history of underemployment. Assist Veteran families to access the community resources necessary to meet their individualized permanent housing needs including, but not limit to, mental health, medical, financial, educational, vocational, residential and social support service. Monitors and documents consumer progress and outcomes.

The Case Manager will have to make frequent visits to the homes and various living situations of the Veterans, in order to assess that living situation, provide support, training and case management services, as needed. This will include both BaltimoreCity and Baltimore Countylocations.

This individual will develop and access community-based contacts and services for Veterans. It may include contacting these services, taking an individual to appointments or enabling the person in other ways to access crucial services.

The Case Manager will train and actively help the individual in daily living skills to include personal hygiene, housekeeping, budgeting, social and interpersonal functioning, skills, etc., and address areas around chemical use recovery, mental health, medical, emotional needs, education, employment, income, etc.

The goal of this service is rapid re-housing for Veterans experiencing homelessness and stability for these families and individuals, as well as supports to assure those Veterans vulnerably housed can maintain that housing and gain stability in housing and in other areas of their functioning.

Essential Functions:

1. Utilize counseling and communication skills to engage Veteran families and build positive working relationships;

2. Act as liaison between the Veteran, Project PLASE and relevant community-based organizations, services, health facilities, etc.;

3. Conduct community-based outreach and intake to identify and market services to eligible program participants;

4. Complete an initial service plan within 5 days after initial assessment and update the service plan at lease every 3 months;

5. Implement that service plan with and from the Veteran and family and

6. Establish a file for each client that includes all documents, contacts and information related to that individual;

7. Actively provide linkage to community agencies;

8. Provide transportation for clients to various appointments, meetings and functional/leisure time activities;

9. Conduct Veteran assessments, evaluations, completing from, narratives, referrals, in process of creating and implementing service plans/agreements;

10. Assist client to obtain and maintain all entitlements and benefits for which they are eligible, including VBA, HUD-VASH, MA, SSI, and /or SSDI, Section 8 housing, Pharmacy Assistance, unemployment, alimony, TANF, food stamps, etc.;

11. Request financial assistance according the DVAfederal rule , to help Veterans meet their goals and maintain housing stability;

12. Work as part of a team with other PLASE, HHVF staff , interns and volunteers;

13. Work closely with Peer Veteran Advocate

14. Help clients and/or family to continue to grow, meeting goals and prevent any further homelessness;

15. Attend continuing education courses, workshops and seminars;

16. Perform other job related duties as needed or assigned.

Required Knowledge, Skills and Abilities:

1. Ability to gain thorough knowledge of Project PLASE’s policies and procedures;

2. Ability to act as a representative of Project PLASE to the public;

3. Ability to effectively communicate empathize with individuals from diverse backgrounds especially Veterans and homeless citizens those with special needs or with a tendency to become hostile;

4. Ability to create and maintain trust and respect when working with clients;

5. Ability to create and maintain effective working relationships with other staff, professionals and paraprofessionals;

6. Ability to work independently and adjust to situations as they arise;

7. Ability to use the computer and complete computer files from client and from data reports;

8. Ability to remain emotionally stable and be creative and compassionate in service delivery;

9. Ability to prepare and maintain accurate records, including work deadlines;

10. Ability to use sound judgment to remain calm in crisis or emergency situations.

11. Ability to see larger picture and goal but also able to work in detail and concrete to assure good quality professional service and in a timely manner.

Education and Experience:

Must have at least a Master’s degree in the Human Services field, preferably Masters in Social Work and 1 year of employment in the field of Human Services Must be willing to work flexible hours and use personal vehicle to deliver services.

Preference given to applicants who are Veterans

Physical and Environmental Conditions:

Work requires light physical effort in the handling of light materials or boxes in non-strenuous work positions of up to 30 pounds and/or continual standing or walking 60 % of the time.

Required to operate a motor vehicle.

Work environment involves everyday risks or discomforts which require normal safety precautions typical of such places as offices, meeting and training rooms, residences, or vehicles, e.g. use of safe work place practices, avoidance of trips and falls, observance of fire regulations and safely operating a motor vehicle.

May be exposed to aggressive human behavior.
