JOB OVERVIEW
· Refer and collaborate with community-based organizations to ensure families receive job training, mental health services, substance abuse services, and other needed services to help families become responsible household members.
· Maintain confidential and secure recording of families’ records and other report information.
· A goal of all resident families is to move from transitional housing at Project L.I.F.E. to permanent housing in the community. Therefore, the case manager will:
· Carry out housing/aftercare specialist responsibilities #1 through #4 for families assigned, and
· Consult with the director frequently for a “second opinion” on professional judgments
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Respect, support, and supervision for every member of every resident family is the core mission of Project L.I.F.E. The case manager and senior case manager are key contributors to that mission; they are the professionals who provide continuing points of contact with the family. These duties and responsibilities are not limited to other assignments that the administration may add. The case manager will:
· Accompany assigned families through the entrance and orientation process;
· Assure that each family is familiar with, and understands, residence rules and procedures.
· Develop a specific Service Plan with each assigned family and review/revise it with them no less frequently than every two weeks.
· Meet weekly with members of each assigned family to evaluate progress in implementing that plan;
· Provide ongoing supportive counseling, information, and referral for assigned families.
· Assist and advocate for assigned families as they negotiate the service delivery system, seek entitlements, etc.;
· Compile case histories for each assigned family; include contacts with health providers, community agencies, and other service agencies.
· conduct daily apartment inspections
· Provide and document training for individuals who are struggling with maintaining clean apartments.
· Summarize these case histories for supervisory and case conferences; note input of other staff.
· Maintain a daily log of all contacts - families, other staff, other agencies, visitors, volunteers, etc.;
· Review the family care monitors’ daily log references to assigned families; enter the same in appropriate case histories; sign off after review.
· Complete all service plans and progress notes by the next business day.
·Will form relationships with area resources, shelters, and landlords to secure stable housing options for shelter clients
· Provide direct care, including but not limited to independent living skills, employment assistance, budgeting, on-time rental payments, and transitional skills, for clients.
Support and supervision for resident families at Project L.I.F.E. is a cooperative endeavor. The case managers will:
· Cooperate with other staff at Project L.I.F.E. and with other service providers in the community to supply comprehensive, family-oriented care;
· Provide night and weekend coverage as required;
· Participate in weekly staff meetings, in-service training opportunities, and weekly review with the executive director.
Qualifications
· Bachelor’s degree in human resources or related field, or a minimum of one year of related experience.
· Demonstrate knowledge and experience with area services, community service, and resources.
· minimum of two years of direct service experience
· Familiar with Homeless Shelter Families
· Knowledge of the Awards/Cares HMIS system
· commitment to operating within a homeless shelter workplace that demonstrates a balance of kindness, competence, and care toward self, colleagues, and the population served.
· Proficiency in a Windows-based computer environment and Microsoft Office Suite
· good verbal and written communication skills
· Possess a valid driver’s license and reliable transportation to get to and from work shifts.
Hours: Third Shift
1:00 pm – 10 pm
Job Type: Full-time
Pay: $22.00 - $26.00 per hour
Expected hours: 40 per week
