What are the responsibilities and job description for the Case Manager - Evenings - Outreach position at Project Hospitality?
Work Schedule: Monday through Friday 3pm to 11pm
Summary:
The Case Manager will work within our Outreach and Housing Placement Program, providing outreach to homeless people living in or near the North Shore area of Staten Island and responding to community calls regarding outreach to other areas of Staten Island.
Responsibilities:
- Connect with homeless people living on Staten Island. Make assessments and appropriate referrals including psych evaluations, TB and HIV testing, detox, etc., complete formal assessment whenever possible.
- Assists clients in filling out forms for entitlements, etc. Complete psychosocial and social history when applicable.
- Counsel clients leading to interim, transitional, and permanent housing or shelter placements
- Complete all paperwork as well as accurate timely statistics as required by the program
- Respond in a timely way to calls from the community regarding homeless individuals in all areas of Staten Island
- Drive and escort clients to the necessary services they require (ESU, detox, etc) using the Outreach van
- Participate in weekly supervision, case conferences, and staff meetings
Qualifications Needed:
- High School Diploma/GED required, Bachelor's Degree preferred
- 1 to 2 years’ experience with the homeless population as well as substance abuse and mentally ill populations is needed
- Excellent client engagement skills are a must
- Must possess a clean and valid driver's license and the ability to drive a passenger van
- Bilingual Spanish/English preferred.
- Must have excellent communication skills both orally and in writing with individuals and groups in regard to complex and sensitive issues.
- The physical ability to perform essential job functions is needed
In addition, the Outreach Case Manager shall perform any duties as required to meet the needs of the program and the clients we serve.
