Project FoodBox is expanding our Food as Medicine programs across New York, connecting Medicaid members and underserved communities with medically tailored groceries, meals, pantry boxes, and critical social care resources.
We are hiring a Community Outreach Event Coordinator to support community engagement efforts throughout Nassau County and help individuals and families access nutrition and social care services that improve health outcomes.
This is not a traditional office-based outreach role.
This person will represent Project FoodBox directly in the community β at health fairs, outreach activations, partner meetings, and local events β building trusted relationships and helping members navigate available resources and programs.
The role includes:
β Community outreach & engagement
Representing Project FoodBox at community events, conducting screenings, educating members on available services, and increasing awareness of Food as Medicine programs.
β Member navigation & support
Helping community members connect to medically tailored food programs, housing resources, transportation support, financial assistance, and other social care services.
β Partnership development
Building relationships with community organizations, clinics, agencies, and local stakeholders to strengthen referral pathways and community impact.
β Outreach operations & reporting
Tracking outreach activities, referrals, event participation, and outcomes while collaborating cross-functionally to support a seamless member experience.
Weβre looking for someone who is comfortable speaking with community members one moment, presenting to partner organizations the next, and helping coordinate meaningful follow-through across teams and programs.
Ideal background:
β’ 2β4 years in community outreach, care coordination, member navigation, social services, or healthcare/community-based programs
β’ Experience working with underserved populations in community settings
β’ Strong communication, relationship-building, and presentation skills
β’ Comfortable conducting screenings, assessments, and community education
β’ Highly organized with strong follow-through and time management
β’ Ability to work independently in a fast-paced environment
β’ Experience with CRM or reporting tools preferred
β’ Bilingual Spanish/English preferred
β’ Valid driverβs license and reliable transportation required
