Today Project FIND operatesfoursupportive housing residences that are home to about700 people,fourOlder Adult Centerswith over 3,000members,acase management program that provides assistive services toover 300older adultsand a program that assists over 100 homeless individuals who are often street homeless.Our members and residents range from the healthy and active to the frail and homebound to the homeless. For all these men and women, Project FIND is a critical resource providing housing, meals,supportand programs that help individuals navigate the challenges of aging by encouraging community engagement and healthy living. Please visit our website at:www.projectfind.org.Job Summary:The Social Worker will work withtenants who areolder adultsat theWoodstock Hotellocation. The program provides case management services, information, referrals, supportive counseling, housingassistance, group work and crisis intervention for clients and their families. The Social Workeris responsible forconducting detailed assessments todetermineclient needs and eligibility for services andengaging withclients to develop goal-oriented service plans.Responsibilities include:Conduct intake and assessment screening, assessing mental and physical health needs and supports, and financial eligibility for benefits, entitlements; conduct home,officeand community visits to address unmet needs; conduct interviews of shelter clients for tenancy.Implement constructive short and long-term interventions through counselingeducation about counseling and physical healthsupports, long-term planning, andassistancewith benefits and entitlements.Provide orientation, support, and services necessary toassisttenants to remain permanently housed in a safe environment.In collaboration with Property Management,assistwith incomeaffidavits,housing applications and recertifications andassistin collecting rentas needed.Work closely with all departments within the agency, including property management, maintenance, housekeeping, and the Older Adult Center, to ensure comprehensive support for tenants. Additionally, be able to coordinate with collateral contacts, such as healthcare providers, home care agencies, familymembersand other formal and informal caregiversto promote tenants overall health, safety, and well-being.Develop and create training events to educate clients on topics that can help support them in their daily lives (e.g., self-care, health and wellness, legal planning).Lead one or more support groups, providing emotional support and psychoeducation about various issues related to aging in place, health and wellness,end-of lifeplanning, mental health, substance use, cognitivewell-beingand self-care.Providetimelydocumentation and maintenance of client files and progress notes.Aftersix months,assistwith training and support ofnew staffonessential job functions, asrequested.Provide supervision, training, and support to paraprofessional staff, including students.Perform other related tasks as requested or assigned.Qualifications:Masters degree in social work, human behavior, gerontology or related required;LMSWstronglypreferred.At leasttwoyearsrelevant experience in social servicesandknowledge of benefits and entitlements a plus.Commitment to Project FINDs mission.Experience working with older adult populations and families preferred.Bilingual language Spanish fluency preferred.Ability to provideaccurateandtimelydocumentationrequired.Time management skills to coordinate multiple responsibilities and services.Ability to work independently and as part of a team, in both a home and office environment.Maintain client and agency confidentiality.Excellent interpersonal, public speaking, and written communication skills. Must be able tocommunicate effectivelyand appropriately with other team members, communityagencies, andclients with diverse opinions, values, characteristics, and cultures. Be skilled in conflict mediation/negotiation and have an assertive approach to problem solving.Excellent oral and written communication skills.Ability toutilizevarious computer programs, specifically Microsoft Word and Excel.Be creative and flexible.Hours: Monday- Friday 9:00am 5:00pm.Project FIND participates in E-Verify, which confirms employment authorization through an electronic database maintained by the Social Security Administration and Department of Homeland Security. The E-Verify process is completed in conjunction with the Form I-9 Employment Eligibility Verification. E-Verify is not used as a tool to pre-screen candidates.

Salary : $57,000 - $77,000
