PRN Clinical Intake Coordinator

Department: Home Health & Hospice

Reports To: Hospice Branch Director

The PRN Clinical Intake Coordinator serves as a liaison between clinical operations and referral sources to support timely, appropriate patient care. This role helps build and maintain community relationships, provides education about hospice services, and supports patients, families, physicians, case managers, discharge planners, social workers, and other healthcare partners throughout the intake and referral process.

This position works closely with Branch Directors, Administrators, Intake Staff, Hospice Physicians, Patients and Families, the General Public, Healthcare Facilities Hospital Palliative Care Units, Hospital Case Management teams and Care Transitions Coordinators.

Qualifications/Educations

  • Required: Associate degree in Nursing.
  • Preferred: Bachelor’s degree in Business, Nursing, Marketing, or a healthcare-related field.
  • Formal sales training is preferred.

Experience

  • Required: At least one year of hospice nursing experience or one year of hospice sales/marketing experience.
  • Preferred: Experience admitting hospice patients to all levels of hospice care, including General Inpatient (GIP).

Licenses, Certifications and Requirements

  • Current Alabama driver’s license.
  • Current automobile liability insurance.
  • Current nursing license.
  • Reliable transportation.
  • Cell phone.

Knowledge, Skills And Abilities

  • Knowledge of Medicare and other payer guidelines related to hospice services.
  • Strong communication, relationship-building, and public education skills.
  • Ability to identify problems, develop solutions, and support strategic planning.
  • Working knowledge of sales principles and community outreach.
  • Understanding of geriatric community needs and referral patterns.
  • Knowledge of physician, hospital, skilled nursing facility, case management, and discharge planning needs.
  • Ability to communicate hospice services effectively across all levels of the healthcare industry.
  • Ability to manage multiple priorities, meet deadlines, and adapt quickly to changing needs.
  • Commitment to teamwork, professionalism, and confidentiality.

Key Responsibilities - Patient Education and Hospice Intake

  • Work with prospective hospice patients and families to assess goals of care and hospice appropriateness.
  • Review home health patients regularly to identify potential transitions to hospice services.
  • Provide weekend hospice agency-on-call support as needed.
  • Manage weekend referrals, complete follow-up on active referrals, and update the Admissions Coordinator and Branch Directors by 8:00 a.m. on Monday.

Territory Management

  • Manage the assigned territory efficiently by promoting awareness and education about hospice services.
  • Develop and execute strategic plans to support territory growth.
  • Identify new referral sources and maximize opportunities to expand hospice services.
  • Maintain consistent territory coverage on a weekly, monthly, and quarterly basis.
  • Use referral tracking systems accurately and maintain current knowledge of all applicable systems.
  • Collaborate closely with Care Transition Coordinators assigned to the same referral sources.

Referral Source and Market Knowledge

  • Maintain knowledge of referral source operations, business practices, and organizational structure.
  • Identify key decision-makers and understand referral source needs.
  • Monitor market conditions, trends, opportunities, and challenges.
  • Assess market potential and distinguish opportunities among physicians, hospitals, and facilities.
  • Maintain awareness of competitive hospice services, including sales efforts and market share.

Administrative Responsibilities

  • Attend meetings promptly and participate actively.
  • Complete assigned tasks accurately and on time.
  • Document calls in RSL by 8:00 a.m. each Monday for the previous week’s activity.
  • Submit expenses, timesheets, and mileage by established deadlines.
  • Follow up on all pending referrals.
  • Update SNF charts weekly and as needed.
  • Perform other duties as assigned.

Working Conditions

This role requires travel by car to physician offices, hospitals, community centers, and events, including exposure to varying weather conditions. Work environments may be unpredictable and are not always under the agency’s control.

Physical Requirements

  • Ability to perform work that requires sitting, standing, walking, and driving.
  • Ability to maintain the vision, hearing, and dexterity necessary to operate a motor vehicle safely.
  • Ability to climb stairs, use elevators, and maneuver across varying types of terrain.
  • Ability to communicate effectively both orally and in writing, including in person and by telephone.
  • Ability to exert up to 50 pounds occasionally, up to 25 pounds frequently, and up to 10 pounds constantly when moving objects or materials.
