Schedule Details: Monday-Friday 0800-1630- Will provide support for Home and Inpatient Hospice patients. Role is part time home hospice community visits and part time inpatient hospice at AGH On call required once every 8 weeks for a full week 1630-0800 M-F and 0800-0800 Sat and Sunday

Our licensed social workers offer assistance with community resources, financial assistance and supportive care and counseling to patients and families.
What You Will Do:
Responsible for the timely effective and efficient identification and counseling of patient(s) and their significant other(s) who require psycho-social, emotional and financial counseling in his or her assigned medical center area(s). This worker performs a variety of social work functions in accordance with the accepted principals and standards of the Social Work Practice
What You Will Need:
- Licensed Social Worker (WI)
- Masters Degree in Social Work
