For over a decade, we’ve been proud to serve our communities with compassionate, patient-first support through licensed clinicians, therapists, and care teams.
We are currently expanding into Monterey County and seeking dedicated healthcare professionals to join our founding team as we launch this new service area.
Open Roles Include:
- Full-Time Home Health RN
- Licensed Vocational Nurse (LVN)
- Physical Therapist (PT)
- Occupational Therapist (PT/PD OT)
- Speech Therapist (ST)
- Medical Social Worker (MSW)
Role Description:
As a member of the home health team, you will be responsible for delivering comprehensive in-home patient care. This includes medication administration, wound care, skilled nursing services, rehabilitation, and family education. You’ll collaborate closely with other healthcare professionals to ensure coordinated, high-quality care tailored to each patient’s needs.
Qualifications:
- Active and valid California license in your field (RN, LVN, PT, OT, ST, or MSW)
- Previous experience in Home Health, Hospice, or Palliative Care preferred
- Strong communication and interpersonal skills
- Empathy, professionalism, and a commitment to patient-centered care
- Valid driver’s license and reliable transportation
