Position Summary:

The Licensed Practical Nurse (LPN) performs routine nursing responsibilities in caring for patients within the scope of nursing licensure and guidelines set by state credentialing board in accordance with the facility’s policies and procedures, local/state/federal regulations, and TJC standards. Performs duties in the nursing department as needed.

Required Knowledge, Skills, Licensure, Training & Travel Requirements (if applicable):

Education: Must have an academic diploma from accredited achool or technical college and current licensure in the state of Louisiana. Must maintain CPR and CPI certifications.

Work experience: Internship in a mental health setting working with adults and feriatrics preferred.

Knowledge, Skills & Abilities: Excellent telephone etiquette; strong communication skills (oral and written); ability to in environment with frequent interruptions. Must be computer literate. Knowledge and follow through with program policies and procedures regarding infection control and sanitation. Completes assignments accurately, in a timely manner, and within procedural guidelines. Organizes and prioritizes work.

Interpersonal Skills: Possess excellent interpersonal skills to help facilitate the development of rapport with patients and their family members, referral sources; and to work cooperatively with in-house physicians, and administrative, clinical and support staff members. Must have tact and be cooperative. Must take initiative and have excellent organizational and time management skills. Displays self-direction by seeking solutions to problems. Seeks out and offers new ideas for improving processes. Deals constructively with stress.

General Performance: Displays willingness to help others. Uses appropriate customer etiquette, e.g., telephone, conversation, demeanor, etc. Takes directions well, accepts constructive criticism and responds accordingly. Respects customer’s, co-worker’s, and organization’s confidentiality. Follow policies/procedures such as appearance, dress, safety, appropriate patient/staff interactions, etc. Works positively with both internal and external customers. Essential Job Duties

1.

Provide basic nursing care within the scope of practice and in compliance with aforementioned policies, procedures and standards.

  • Perform the following duties with accuracy including but not limited to: dressing changes, medication administration/observation, and documentation.
  • Assist RN’s with nurse’s assessments and treatment plans as directed.
  • Ensure that lab work and x-rays are done as ordered and reported to the Charge nurse/Physician.
  • Perform admissions and discharges of patients according to policies and procedures of the facility.
  • Perform assessments of patient’s medical needs and reports this information to his/her RN/physician in a timely manner.
  • Perform patient care as directed and document accurately and concisely on patient’s behavior and participation in treatment.
  • Document behavioral changes and report these to the RN/physician in a timely manner.
  • Provide quality patient care by complying with physician’s orders, counselor directives, and treatment plans.
  • Conduct group/individual interactions for patients and families focusing on areas of medical and psychiatric health education and skill acquisition for patients.
  • Notify Charge Nurse/Nurse Manager of staffing needs, patient/family needs and concerns in a timely manner.

Accountable for storage, count and administration.

  • Administer prescribed medications as ordered by physician.
  • Maintain clinical competencies specific to Adult/Geriatric/Medical Psychiatric population.
  • Transcribe physician orders accurately.
  • Participate in weekly treatment team meetings and daily treatment planning meetings as directed by supervisor.

3.

Participates as an active member of multidisciplinary treatment teams ongoing development, implementation, and evaluation of patients’ treatment plans.

  • Attend and communicate pertinent information at morning staffing and treatment team meetings.
  • Assure effective documentation in patient’s records to reflect patients’ progress and other pertinent data.
  • Assist with staff orientation and training to department.
  • Perform chart audits as requested.

4.

Maintain an appropriate knowledge of Psychiatric theories and practices and apply this knowledge in patient care and treatment where indicated.

  • Attend all in-service trainings and completes the required annual tests to remain in compliance with hospital and TJC educational requirements (i.e., body mechanics, infection control.)
  • Demonstrates the ability to carry out responsibilities for fire drills, codes, orientation of new staff, and other areas as directed.
  • Keep the following certifications current: CPR, CPI, and nurse licensure.
