Qualifications
- Current Oklahoma LPN license
- Home health experience preferred
- Recent direct patient care experience preferred
- Valid driver’s license, reliable vehicle, and auto insurance
- Excellent verbal and written communication skills
Responsibilities
- Coordinate care across disciplines to ensure optimal patient outcomes
- Communicate changes in patient condition to physicians and case team members
- Implement and update physician orders and care plans
- Maintain accurate, timely (within 48 hours), and complete documentation
- Ensure compliance with Medicare guidelines and company policies
- Participate in on-call, holiday, and weekend rotation as assigned
- Support regulatory readiness and emergency preparedness efforts
- Assist with orientation and training of new staff as needed
Physical & Functional Requirements
- Ability to travel throughout the service area
- Ability to lift up to 50 lbs and assist with patient care
- Maintain focus during frequent interruptions
- Strong observational and communication skills
