Job Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) for our Hospice team, offering both full-time and part-time positions with flexible daily schedules. The LPN will operate a company vehicle to provide compassionate, patient-centered care in the home setting. This role requires a proactive approach to patient assessment, symptom management, and coordination with interdisciplinary teams to ensure comfort and dignity for our hospice patients.
Responsibilities
- Provide comprehensive nursing care to hospice patients in their homes, including medication administration, wound care, and symptom management.
- Conduct patient assessments and monitor vital signs regularly to evaluate health status and identify changes requiring intervention.
- Collaborate with physicians, social workers, and other healthcare professionals to develop and update individualized care plans.
- Maintain accurate medical documentation within electronic health record (EHR) systems.
- Educate patients and family members on disease processes, medication management, and comfort measures related to hospice care.
Experience
Candidates should possess prior experience in hospice or palliative care settings with a strong foundation in critical care principles. Experience working with diverse populations is highly valued. Proficiency in EMR/EHR systems. Additional experience in areas of pain management, long-term care facilities, skilled nursing facilities and hospital medicine, will enhance candidacy. Knowledge of infection control practices, medical documentation standards, and familiarity with home & community care protocols are advantageous.
