Job Overview

We are seeking a dedicated and experienced Licensed Practical Nurse (LPN) for full-time hospice care. Join our team as an LPN Hospice Full-Time caregiver and help provide comfort and dignity to those at the end of their journey through expert nursing support combined with heartfelt compassion! The LPN will be responsible for delivering high-quality patient care, coordinating with interdisciplinary teams, and ensuring compliance with healthcare standards. Full Time, Monday through Friday, flexible schedule, no call. Attend IDG meeting every other Thursday.

Duties

  • Provide direct nursing care to hospice patients in their homes, including assessments, medication administration, and symptom management.
  • Monitor vital signs and evaluate patient conditions regularly to identify changes requiring intervention.
  • Assist with complex procedures such as IV infusions, catheterizations, and wound care while adhering to aseptic techniques.
  • Educate patients and families on disease processes, medication management, and end-of-life care options.
  • Document patient interactions accurately using electronic health records (EHR/EHR systems) while maintaining strict confidentiality policies.
  • Collaborate with physicians, social workers, and other healthcare team members to develop and implement individualized care plans.
  • Ensure compliance with all long-term care regulations and hospice standards while maintaining a compassionate approach.

Requirements

  • Valid LPN license, with current CPR certification.
  • Proficiency with EMR/EHR systems. Experience with medical documentation.
  • Experience working with diverse patient populations including geriatric patients, individuals with developmental disabilities, Alzheimer's disease, dementia, or memory impairments.
  • Ability to perform physical examinations and assessments accurately; skilled in vital signs measurement and client communication.
  • Strong organizational skills with attention to detail; ability to work independently while managing multiple patient cases effectively.
