Position: Registered Nurse in Total Quality Management

Key Responsibilities
- Improve patient outcomes by coordinating care across hospital and outpatient settings.
- Reduce readmission rates through clinical monitoring and early intervention.
- Empower patients via personalized health coaching and education.
- Coordinate care plans among providers and community resources, addressing social determinants and medication management.
- Transform patient participation in recovery, enhancing quality metrics.
- Licensed Registered Nurse with valid credential from the Oklahoma Board of Nursing.
- BLS certification preferred (American Heart Association or Red Cross).
- Willingness to obtain a compact license within 90 days.
- Prior experience as Care Manager, Case Manager, or Patient Navigator in ambulatory or home health settings is preferred.
