Role Description
We are seeking a dedicated and compassionate Field Case Manager with an RN license to join our team in a full-time remote capacity. The Field Case Manager will be responsible for assessing and managing patient care needs, developing comprehensive action plans, coordinating medical services, and collaborating with healthcare providers to ensure optimal patient outcomes. Additionally, the role involves discharge planning, utilization management, and effective communication with patients, families, and stakeholders to support recovery and rehabilitation processes.
Qualifications
- Strong skills in Case Management, including assessment, planning, and care coordination
- Proficiency in Discharge Planning and Utilization Management to ensure effective transitions and cost efficiency
- Valid RN license
- Excellent communication, problem-solving, and time management skills
- Ability to work independently in a remote capacity and adapt to varying client needs
- Experience in Workers' Compensation or healthcare case management is preferred
- Proficiency in using case management systems and technology tools
