Overview
We are seeking a highly skilled and compassionate Registered Nurse to join our healthcare team. The ideal candidate will have extensive experience in various clinical settings, including acute care, ICU, emergency medicine, geriatrics, and outpatient services. This role offers an opportunity to provide exceptional patient care across diverse medical disciplines, utilizing advanced skills in medical documentation, EMR/EHR systems, and specialized procedures such as IV infusion, suturing, catheterization, and ventilator management. Candidates with experience in hospital medicine, long-term care, hospice & palliative medicine, and working with special populations such as pediatric or geriatric patients are encouraged to apply. Duties
- Perform home health visit, including assessment, SOC.
- Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations.
- Administer medications and injections accurately while adhering to aseptic techniques and sterile processing protocols.
- Manage airway support including ventilator management and oxygen therapy; perform advanced procedures such as tube feeding and gastric lavage when necessary.
- Document all patient interactions thoroughly using EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks to ensure accurate medical records.
- Coordinate diagnostic evaluations such as sonography, laboratory testing, radiology imaging (mammography), and specimen collection/processing.
- Provide specialized care including wound suturing, ostomy care, dialysis management, IV insertion/infusion, catheter care/catheterization procedures, and post-operative discharge planning.
- Deliver patient education on health coaching topics such as nutrition, pain management, medication administration, and disease prevention strategies.
- Assist in complex cases involving trauma medicine (Level I & II Trauma Centers), infectious disease care, or critical care transport situations.
- Ensure infection control standards are maintained at all times; implement appropriate isolation precautions when needed.
- Collaborate with multidisciplinary teams to develop individualized care plans for patients with chronic conditions like Alzheimer’s/dementia or developmental disabilities.

- Experience working with diverse populations including pediatrics (neonatal ventilator use), geriatrics (memory/dementia care), behavioral health patients, or individuals with disabilities.
- Proficiency in managing complex cases involving acute pain management, trauma medicine, infectious diseases, or post-acute care scenarios such as sub-acute or long-term care facilities.
- Skilled in using EMR/EHR systems like Epic, Cerner, Athenahealth for documentation; familiarity with laboratory experience and diagnostic evaluation processes is advantageous.
- Additional certifications such as Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS), sterile processing training or esthetic laser treatment knowledge are a plus but not required.
