Job Overview
We are seeking compassionate and dedicated Home Health Aides to provide high-quality in-home care to clients with diverse needs. This role involves supporting patients with daily activities, ensuring their safety, and promoting independence within their homes or community settings. Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor and observe patient health status, including vital signs and overall well-being
- Support patients with memory care needs, including Alzheimer's and dementia care
- Assist with medical procedures such as catheter care, Hoyer lift transfers, and hospice-related tasks
- Prepare meals and assist with nutritional needs while adhering to dietary restrictions
- Perform light housekeeping tasks including laundry and cleaning to maintain a safe environment
- Document patient progress and communicate effectively with healthcare teams while maintaining HIPAA compliance
- Provide emotional support and companionship to patients, especially those with developmental disabilities or in long-term care settings
Skills
- Proven experience in patient observation, home & community care, or long-term care environments
- Knowledge of medical terminology and medical documentation practices
- Ability to work with people with disabilities, developmental disabilities, or cognitive impairments such as Alzheimer's or dementia
- Proficiency in using EMR/EHR systems for accurate record keeping
- CPR certification and experience with medication administration are highly desirable
- Familiarity with assisted living facilities, nursing homes, hospice care, and hospice-related procedures
- Skills in caring for seniors and individuals requiring memory or hospice care
- Ability to assist with ADLs, meal preparation, light cleaning, laundry, and other home care tasks
- Experience working with people who have disabilities or require specialized caregiving support
