What are the responsibilities and job description for the PTA Physical Therapy Assistant - PRN position at Professional Home Health Care?

Professional Home Health Care Colorado Springs is hiring a PRN PTA Therapist to make home visits in the Colorado Springs and/ or Pueblo, and Pueblo West areas. Position is prn, per visit.
Experience in homecare preferred but we do offer a great training program if you are new to the field of homecare. New grads welcome!

Qualification:
- Current Colorado Physical Therapy Assistant license
- Valid Driver’s license, clean MVR, and proof of auto insurance
- Reliable transportation
- Must be able to pass Criminal Background Check
- Assessment skills
- Computer skills
Responsibilities and Duties include:
- Makes home visits and provides care ordered by the physician under a plan established by the PT.
- Assess patients’ condition; reports findings to PT, office and physician; obtains orders for new needs, and implements the PT care plan.
- Completes required documentation that makes evident patient/family needs, interventions, and progress in accordance with clinical, quality, and regulatory requirements.
