What are the responsibilities and job description for the Home Health LVN Field Nurse-Southlake, Texas position at Professional Home Health Care Inc.?
- Assist in identifying the patient's physical, psycho-social, and environmental needs as evidenced by documentation, clinical record, case conference, team reports, and evaluations, under the direction of the RN Case Manager and the Director of Clinical Services
- Assumes responsibility for the physical care of the patient under the overall direction of the RN Case Manager and the Director of Clinical Services, in accordance with the plan of care
- Provides nursing services, treatments, diagnostic and preventive procedures requiring special skills under the direction of an RN and understands that an LVN may only perform those treatments for which (s)he is qualified by training and experience special treatment procedures performed by an LVN will only be approved if documentation of competency has been received by the Director of Clinical Services
- Understands that an LVN may not admit patient
- Understands that an LVN may not conduct the death pronouncement in the' patient's home
- Takes physician's telephone orders and that all written orders require RN co-signature
- Provides and maintains a safe environment for the patient/caregiver
- Provides frequent coordination of care regarding the condition of the patient to the amending physician, the Director of Clinical Services, and other disciplines and service providers involved in patient care
- Documents visits and coordination of care in a timely manner and on an ongoing basis for the clinical record
- Participates in the quality improvement program
- Attends case conferences, team meetings, and in-services
- Provides care utilizing infection control guidelines, and National Patient Safety Goals, to protect both the staff and the patient
- Promotes and adheres to agency philosophy and administrative policies
- continuing education programs for professional growth and competency
- Rotates on-call coverage with other staff nurses, as appropriate

Pay: Up to $41.00 per hour
