Professional Home Health Care Agency, Inc. is committed to providing superior in-home care through a wide range of services including skilled nursing, rehabilitation, medical social services, personal support and homemaker services to our patients in their homes. We have open positions in Case Management for Registered Nurse Nurses in our Whitley branch office.
Job Responsibilities:
The Case Manager is responsible for reviewing medical records to assure quality control and best practices in standards of care. Case Managers assist in providing on-going educational programs on documentation, coverage requirements and other staff development programs and participates in the medical reimbursement process to ensure timely and accurate filing of patient claims.
Job Qualifications: .
- Current RN or LPN license, with no sanctions, restrictions, or provisions, in the state of Kentucky.
- Minimum of Four years of Home Health Care experience.
- Case Management, utilization review, OASIS, Matrix and supervisory experience preferred.
