What are the responsibilities and job description for the RN Case Manager Supervisor position at Professional Case Management Services of America, Inc?
Professional Case-Management Services of America (PCSA) currently has a part-time contract opening.
- Must meet educational requirements and have current licensure as a Registered Nurse in State of Georgia with at least 3 years of professional experience serving individuals with intellectual and developmental disabilities
- Conducts field visits evaluating the quality and outcome of services and identifying unmet needs, and through assessment and evaluation related tasks
- Provides telehealth and face-to-face contact with all newly assigned Intensive Support Coordination (ISC) individuals within the first month of service and at least annually
- Provides clinical consultation to case managers identifying and addressing barriers to care; accessing needed resources and services offered through the waiver and the larger healthcare system; taking active measure to address complex needs; fostering and maintaining family and other informal relationships and supports
- Must have reliable transportation, travel is required
Pay: $75,000.00 - $80,000.00 per year
