Professional Care Medical Practice P.C. is currently looking for a Licensed Social Worker (LMSW or LCSW) or Licensed Mental Health Counselor (LMHC) to work as a Behavioral Health Professional in the medical department at the Fulton County Correctional Facility in Johnstown, NY.
This is a Full-Time positon, 40-hour per week Monday-Friday, during normal business hours (i.e., 8:00a - 4:30p).
In this role you will be responsible for the clinical monitoring, coordinating and implementing of the mental health program in accordance with NCCHC standards and will serve as the mental health liaison between the medical unit and facility staff.

Requirements:
- Must be licensed as a LMHC, LMSW, or LCSW in the State of New York.
- Must hold a valid driver’s license & have reliable transportation.
