Description:
The PRP Counselor is responsible for implementing and monitoring the individualized rehabilitation activities outlined in each client’s Individual Rehabilitation Plan (IRP). The counselor provides direct support services designed to enhance clients’ functioning, promote independence, and assist in achieving treatment goals. The role requires collaboration with therapists, clients, and families to ensure consistent and effective service delivery in compliance with COMAR regulations Code 10.63.03.09(Adults) - 10.63.03.10(Minors).
Requirements:
- Bachelor’s degree in a health-related field; or Associate’s degree in a health-related field; or
- Minimum of 30 college credit hours toward a bachelor’s degree in a health-related field; or
- At least 1 year of work experience in a supervised mental health setting.
- Strong communication, organizational, and documentation skills.
- Ability to maintain confidentiality and adhere to ethical standards.
Key Responsibilities:
Service Delivery:
- Implement rehabilitation activities in accordance with the client’s IRP.
- Conduct and document a minimum of 4 hourly sessions per month for minors and 6 hourly sessions per month for adults (18 ).
- Maintain active engagement with clients, therapists, and parents/guardians to support goal achievement.
Client Contact & Documentation:
- Make initial contact within 48 hours of case assignment; document all attempts.
- Complete progress notes within 24 hours of each session; submit all notes by deadline.
- Submit monthly summaries and all required paperwork by the 5th of each month for the prior month.
- Late submissions may result in delayed reimbursement.
IRP Review & Collaboration:
- Review the IRP with the client and parent/guardian within the first two visits.
- Maintain understanding of diagnosis, goals, and interventions.
- Collaborate with the rehabilitation specialist, therapist, and client 30 days before authorization expires.
Supervision & Training:
- Attend bi-weekly team supervision and all mandatory training sessions.
- Maintain compliance with documentation and supervision requirements as outlined by program policy.
Job Types: Full-time, Part-time
Pay: $23.20 - $27.94 per hour
Expected hours: No more than 40 per week
