SummaryThe Nurse Coordinator is responsible for supervising and implementing an array of services to enrich the lives of people with varied disabilities. Services include, but are not limited to, case management, transportation, the development of social relationships, and providing support in the areas of recreational, therapeutic, and personal living skills. QUALIFICATIONSHigh School degree required, college degree preferred. Current licensure by the Commonwealth of Massachusetts as a Licensed Practical NurseThree years supervisory experience working with people with disabilities and ABI experience preferred but not required.Acceptable Criminal Offender Record Information (CORI) and fingerprint check.Acceptable screen against the OIG-LEIE (Office of the Inspector General’s List of Excluded Individuals and Entities).Possession of a valid driver’s license and ability to transport residents in agency vehicle and also comply with agreement to use one’s personal vehicle for business purposes.ESSENTIAL DUTIESAssist Director of Specialized Services in overseeing the day to day operation of the program: medical appointments, hospital discharges, transitional careProvides feedback to Director of Specialized Services regarding residential staff as appropriate. Participate in interviewing and hiring LPN’s.Monitor general health status of individuals, screen individual’s complaints of illness and recommend appropriate action as required.Attend and participate in ISP/Transition meetings as needed.Monitor and implement service plans throughout the day including medical, behavioral and therapeutic requirements.Assist with case management responsibilities and/or service documentation requirements as assigned and defined by the Director of Specialized Services.Facilitate monthly staff and house meetings, in conjunction with the Director of Specialized Services.Ensure appropriate medical equipment is available as needed.Attend required in-services and provide in-services to staff as needed.Communicate with program LPN’s, physicians, pharmacists, other medical providers as appropriate; document as required.Perform all duties in conformance to, or in recognition of existing Agency policies and procedures including the Employee Handbook.Schedule and attend medical appointments as necessary, document as required.Monitor individual’s medical needs and doctor’s orders; oversee and ensure diets are in place in accordance with medical orders.Administer medications and monitor treatment plans in residences/day center.Ensure proper storage, disposal and documentation of all medications in residences/day centersAttend discharge meetings as needed and ensure appropriate follow-up.Assist the Director of Health Services in scheduling LPN coverage including on-call coverage, as requested.Communicate with residents, family members, guardians and residential staff on relevant health issues. Such communications should be coordinated with the Director of Specialized Services.Assume various other and related duties as assigned by the Director of Specialized Services and the Director of Health Services.Confer with physicians, dentists, and therapists as appropriate.Communicate with/provide necessary information on relevant health issues to parents, guardians, other providers and applicable Agency staff.Respond to participant complaints of illness and recommend appropriate action.Confer with pharmacists regarding medications and their acquisitions as appropriate.Monitor seizure disorder records.Monitor general health status of participants, including hygiene issues and skin integrity.Complete all health related documentation requirements.Company DescriptionFor more than 50 years, this 501(c)(3) nonprofit organization has supported individuals with intellectual and developmental disabilities and brain injuries through a diverse menu of programs and clinical services, including residential, clinical and health services, employment supports, other specialized services, and a variety of day program activities.
