Overview
We are seeking a compassionate and dedicated Nurse Practitioner (NP) to join our healthcare team. The ideal candidate will provide essential nursing care to patients, ensuring their comfort and well-being while adhering to established protocols and standards. This role requires a strong commitment to patient care, effective communication skills, and the ability to work collaboratively within a multidisciplinary team.
Responsibilities
- Prescribe Medication to patients at Skilled Nursing Facilities, Assisted Living, and Home.
- Monitor patient vital signs and report any changes in condition.
- Evaluate and create a plan of care for our geriatric patients.
- Provide emergency care as needed, utilizing skills in emergency medicine.
- Conduct case management duties, coordinating patient care plans with healthcare providers.
- Create H&P's, Progress notes for each pt seen my you, and follow facility protocal and regulations on notes done.
- Document patient information accurately using electronic health record systems such as Cerner.
- Offer support and care for individuals with developmental disabilities and dementia.
- Schedule your own visits based on your availablity.
- Maintain a clean and organized work area while adhering to safety protocols.
Experience
Candidates should possess the following skills and experience:
- Proven experience in emergency medicine or similar healthcare settings.
- Familiarity with case management processes.
- Experience in creating H&P's and Progress notes in EMR.
- Able to prescribe and adjust patient medication if needed.
- Background in working with people with developmental disabilities.
- Knowledge of dementia care practices is advantageous.
- Experience in geriatric patients is a plus.
- Understanding of aliment-related health issues is beneficial.
- Familiarity with electronic health record systems, EMR, is desirable.
