About the Role
We are a growing Physical Medicine & Rehabilitation (PM&R) and Pain Medicine practice seeking a motivated Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in a Skilled Nursing Facility (SNF) setting.

Key Responsibilities
- Evaluate and manage patients within skilled nursing facilities
- Diagnose and treat conditions including:
- Functional decline
- Musculoskeletal pain
- Spasticity
- Neuropathy
- Post-hospital rehabilitation needs
- Perform bedside procedures (training provided)
- Conduct ultrasound-guided injections (training provided)
- Collaborate with interdisciplinary teams (PT/OT/ST)
- Maintain accurate and timely EMR documentation
- Work closely with supervising PM&R and Pain Medicine physicians
Qualifications
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA)
- New graduates are encouraged to apply – training provided
- Interest in PM&R, Pain Medicine, Orthopedics, or Geriatrics
- Ability to thrive in a fast-paced, high-volume environment
- Strong interpersonal and communication skills
- Procedural interest preferred (not required)
- SNF experience is a plus, but not required
