Team Lead- Patient Care Coordination
About the Role
We’re looking for a motivated Team Lead to manage a team of Patient Care Coordinators (PCCs). In this role, you’ll help recruit your team, coach and mentor your team, oversee day-to-day operations, and ensure quality care for patients. You’ll also play a critical role in maintaining compliance with HIPAA and company standards, while helping shape a culture of accountability and growth.
What You’ll Do
- Lead, mentor, and support a team of PCCs to achieve daily performance goals.
- Monitor productivity, quality, and compliance metrics.
- Ensure all patient care activities meet HIPAA and confidentiality requirements.
- Provide coaching, feedback, and career development opportunities.
- Collaborate with referral partners and internal teams to ensure smooth patient care operations.
- Drive innovation and initiatives to improve our patient experience.
What We’re Looking For
- Experience in healthcare administration, patient coordination, or a related field.
- Leadership experience (formal or informal).
- Knowledge of HIPAA and compliance standards.
- Strong communication, organizational, and problem-solving skills.
- Individuals who are curious, creative and driven to drive change.
- Bachelor’s degree in a related field preferred (or equivalent experience).
