Role Description
The Direct Service Professional located in Hudson will provide on-going support to participants enrolled in enrichment programming, life skills classes and/or at the work place. These supports are provided at either ProAct or in the community. Reporting and fact-to-face counseling are also responsibilities of the Direct Service Professional.
Supports individuals with disabilities while interacting in the community. Plans and prepares class curriculum/lesson plans to fit individual needs and teaches classes as required. Provides occasional job coaching.
Assists with behavior management plans. Completes all record keeping in a timely and accurate manner, including but not limited to consumer case notes, behavior observation charts, seizure logs, daily logbook, and accident/injury reports. Assists with consumer’s personal care needs including but not limited to toileting, feeding, food preparation/set-up, break room supervision, dressing and washing. Provides daily coverage for lunch. Uses a company vehicle to transport individuals.
Qualifications
- High School diploma or general education degree (GED) with further training in health/rehabilitation preferred. Six months related experience and/or training; or equivalent combination of education and experience.
- Successfully complete DHS background study prior to beginning employment.
- CPR and 1st Aid training provided once hired.
- Possess valid driver’s license with acceptable driving record; insurable at non-risk rates.
- Must be able to pass DOT physical.

$20.50 per Hour
