MDS Coordinator

A growing nursing home operator is seeking an experienced MDS Coordinator for a skilled nursing facility in Suffolk County, Long Island.

The MDS Coordinator will oversee the facility's MDS and reimbursement process and work closely with corporate reimbursement leadership to ensure accurate assessments, documentation, compliance, and reimbursement.

Responsibilities

• Manage the MDS process for a small-to-medium-sized skilled nursing facility.
• Ensure timely and accurate completion of MDS assessments.
• Support Medicare/Medicaid reimbursement and PDPM processes.
• Review clinical documentation for accuracy and reimbursement opportunities.
• Coordinate with nursing, therapy, and interdisciplinary teams.
• Ensure compliance with CMS requirements and applicable regulations.
• Monitor assessment schedules and reimbursement-related deadlines.
• Work closely with and report to a Corporate MDS professional.

Qualifications

• RN license required.
• Must have a minimum of 2–3 years MDS experience within skilled nursing/LTC.
• Strong understanding of MDS, PDPM, Medicare/Medicaid reimbursement, and clinical documentation.
• Organized, detail-oriented, and able to collaborate effectively with facility leadership and corporate reimbursement teams.
