Responsibilities
- Deliver comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating physiological data to inform care plans.
- Administer medications and injections accurately following established protocols, including IV infusion and catheter care.
- Assist with medical procedures such as wound suturing, specimen collection, and sterile processing to ensure optimal patient outcomes.
- Manage patient documentation utilizing EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems like Epic, Cerner, Athenahealth, or eClinicalWorks for seamless record-keeping.
- Support patient education initiatives by providing health coaching on topics such as medication management, nutrition, dementia care, Alzheimer's care, and pain management strategies.
- Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and post-acute care services for diverse patient populations.
- Respond swiftly to triage calls and urgent care needs using critical thinking skills honed through experience in emergency medicine or Level I/II trauma centers.
- Possibly some outside sales calling on new clients and representing the zCompany
Requirements
- Valid licensure as a Licensed Practical Nurse (LPN) in the respective state or jurisdiction.
- Proven experience working in various clinical environments such as hospitals (including ICU or PICU), nursing homes, assisted living facilities, or home & community care settings.
- Demonstrated proficiency with EMR/EHR systems like Epic or Cerner; familiarity with ICD-9/ICD-10 coding is highly desirable.
- Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques.
- Experience with IV insertion, medication administration (including injections), tube feeding, and ventilator management preferred.
- Ability to perform physical examinations and vital sign assessments accurately while maintaining infection control standards.
- Excellent communication skills for effective patient assessment and collaboration with healthcare teams; experience in triage or urgent care is a plus.
