We’re looking for Care Coordinators (CTC) to join our growing team and play a key role in supporting the onboarding of patients into our Chronic Care Management (CCM) program.About the RoleAs a CTC, you’ll be responsible for educating patients on CCM services, obtaining consent, creating the first care plan, and ensuring a smooth relationship with your patients as you help then navigate through CCM.
