What are the responsibilities and job description for the Supervising Case Manager - Yates County Office for Aging position at PRO ACTION OF STEUBEN AND YATES INC?
Supervision: Supervise case management as well as housekeeping staff. Monitor contracts with In-Home and Respite vendors to ensure completion of contracted tasks. Monitor programs and vendors as assigned.
Reporting and data collection: Prepare required board and legislative reports, assist with NYSOFA Annual Update. 4 Year Plan and grants as needed.
Case Management: Take referrals from multiple sources, determine eligibility, complete all required assessments, care plans, correspondence, follow up, and other forms. Complete data entry in NYSOFA and Pro Action databases. Conduct home visits, provide information on long term care options, assist with applications as needed and maintain private care registry.
Budget Management: Monitor budgets from contracted companies and service delivery partners. Monitor budgets for programs and funding sources as assigned by Associate Director. Request budget modifications as needed. Prepare subcontractor contracts. Processes invoices for the payment of subcontractor and other program expenses.
Ideal skills
- Experience working in field specific to the job duties.
- Experience working with database systems and tracking systems and a variety of virtual platforms.
- Strong organizational and competent math skills.
- Communication and language skills to comprehend documents and other correspondence.
- Strong computer skills with basic knowledge of Excel Microsoft office and other office equipment such as copier, scanner, fax etc...
- Ability to interact with customers from a variety of backgrounds and with different levels of management.
- Ability to exercise occasional independent judgement.
- Capability to apply common sense understanding to carry out instructions in written and oral form.
- Ability to deal with problems involving several concrete variables in standardized situations.
- Experience in advocacy work.
