Role Summary
We are seeking a Registered Nurse who excels in conducting comprehensive patient assessments to join our care team in the Milford, Delaware area. The RN will play a vital role in evaluating client needs, developing care plans, and supporting our team in delivering exceptional home-based care. This role requires a detail-oriented, compassionate nurse who thrives in both in-home and virtual settings.
Key Responsibilities- Conduct initial and follow-up clinical assessments per agency and Medicaid guidelines
- Evaluate client health status, level of care needed, and support requirements
- Develop individualized care plans based on assessment findings
- Perform in-home visits for assessment and care planning
- Coordinate with caregivers, case managers, and physicians to ensure continuity of care
- Monitor and document client progress and adjust care plans as needed
- Ensure compliance with state regulations and agency policies during all assessments
- Provide education to clients and families regarding services and care expectations
- Participate in quality assurance and clinical review meetings when needed
- Active Registered Nurse (RN) license in the State of Delaware
- At least 1–2 years of experience performing patient assessments (home health, hospice, or long-term care experience preferred)
- Excellent critical thinking and clinical judgment skills
- Proficiency in documentation and electronic health records (EHR)
- Strong interpersonal and communication skills
- Comfortable working independently in the field and remotely as needed
- Valid driver’s license and reliable transportation
- BLS certification required
- BSN preferred but not required
