Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts when necessary
- Monitor patient conditions through patient observation, vital signs measurement, and medical documentation to ensure safety and well-being
- Administer medications accurately following medical records and home health care regulations while maintaining strict patient confidentiality in compliance with HIPAA standards
- Support clients with specialized needs including Alzheimer's care, dementia care, end-of-life care, and hospice services with compassion and professionalism
- Utilize Electronic Health Record (EHR) systems to document medical records, care notes, and client interactions efficiently and accurately
- Assist with meal preparation, light housekeeping, laundry, cleaning, and other home care tasks to promote a safe and comfortable living environment
- Communicate effectively with clients and their families about care plans, progress updates, and any changes in condition to ensure transparency and trust
Qualifications
- Proven experience working as a caregiver in assisted living facilities, nursing homes, or home healthcare settings with a focus on geriatrics or disabilities
- Strong knowledge of medical terminology, medication administration, catheter care, CPR, first aid, and basic life support techniques
- Familiarity with EMR/EHR systems for medical documentation and records management
- Ability to perform patient transfer methods safely using equipment such as Hoyer lifts or other assistive devices
- Experience working with individuals with developmental disabilities or cognitive impairments like Alzheimer's or dementia is highly desirable
- Excellent patient communication skills combined with compassionate caregiving abilities
- Knowledge of home health agency regulations and compliance standards such as HIPAA is essential
- Physical ability to perform heavy lifting when necessary and maintain patient safety during transfers or mobility assistance
