Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ health by observing changes, and reporting findings accurately in accordance with HIPAA regulations
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking
- Support medication reminders following care plans and ensuring proper documentation of medication schedules
- Utilize specialized equipment such as Hoyer lifts and manage heavy lifting tasks safely to ensure client safety and comfort
- Maintain a clean, safe living environment by performing light housekeeping duties including cleaning and laundry
- Provide companionship and emotional support while engaging clients in social activities or community outings
Skills
- Proven experience in home health, assisted living, or long-term care environments with a focus on patient observation and monitoring
- Knowledge of dementia care, Alzheimer’s care, memory care, hospice care, and working with individuals with developmental disabilities or autism spectrum disorder
- Certified in CPR and First Aid; familiar with HIPAA compliance to ensure patient confidentiality
- Strong ability to work with people with disabilities or seniors requiring specialized care such as nursing home or group home experience
- Excellent communication skills for collaborating with healthcare teams, social workers, families, and clients’ support networks
- Ability to perform physical tasks including heavy lifting and the use of assistive devices like Hoyer lifts safely and effectively
- Experience developing or following personalized care plans; adept at assisting with medication management and vital signs monitoring
- Compassionate attitude combined with a proactive approach to patient safety, social engagement, and overall well-being
- Current negative TB test results within the past 12 months or ability to have testing done at PRN Staffers approved urgent care location -cost covered by PRN Staffers
