You are an RN who
- A licensed RN (or eligible for licensure in Wisconsin)
- Passionate about building meaningful connections with patients
- Comfortable working independently while supported by a collaborative team
- Skilled in both written and verbal communication
Key Responsibilities
- Serve as Case Manager, coordinating comprehensive patient care
- Conduct and document thorough assessments; develop individualized treatment plans
- Collaborate with patients, families, and clinical staff to optimize outcomes
- Maintain clear and concise documentation, including discharge summaries
- Educate patients and caregivers on safe discharge planning in coordination with the clinical team
