- PRN Home Care, Inc. is a Medicare certified skilled home health agency.* We are looking for a qualified Registered Nurse who seeks a close-knit, team oriented, client-centered work environment to join us in caring for Brevard County, Florida residents. Our Home Health nursing staff coordinates comprehensive, interdisciplinary nursing care for patients and empowers patients and their loved ones through education and personal goal setting. We provide skilled care in the home on a per visit basis for skilled needs. We are not a companion care agency or have our staff in the home for blocked hours. --- _Full time positions available for Brevard County, Florida. ASAP!!!*_
Job Duties/Responsibilities:
*Assess, plan, evaluate, and implement holistic patient care
*Ensure care needs are being met with an ongoing focus of goal achievement
*Utilize an interdisciplinary approach to patient care that includes providers and family members
* Connecting patients to the community resources as needed and maintain open and direct communication channels
* Excellent assessment skills
* Superior critical thinking and effective communication skills
* Ability to work in a autonomous environment
* Timeliness with documentation in an EMR system and adherence to organizational standards
* Integrity and self-discipline
Requirements:
* Current Registered Nurse license in Florida with 1 year acute care experience
* Reliable transportation
* Current CPR
* COVID-19 Vaccination (preferred)
* Home Care nursing experience preferred but for the right candidate we will be more than willing to train
