Job Summary
We are seeking a dedicated and highly skilled Long-Term Care Registered Nurse to provide comprehensive nursing care within a long-term care facilities. This role will work in vents, rehab, long term care or dementia units. As a key member of the healthcare team, you will deliver patient-centered care, coordinate treatment plans, and ensure compliance with healthcare standards while fostering a compassionate environment for the residents. **RN candidates only - Vent/Trach experience is a MUST**
- Schedule: 12 hours NOC shift, 36 hours/week
- Shift Information: 6p-6:30a
- Weekend Rotation: Every other weekend required
- Holiday requirements: Every other holiday required
- Average Ratio: 1: 9-13 residents (Vents/Rehab); 1: 20 -22 (all other units)
Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and health history documentation.
- Administer medications.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Document all patient interactions accurately in medical records while ensuring compliance with medical terminology standards.
- Participate in discharge planning, case management activities, triage assessments, and emergency response protocols within acute care or hospital medicine settings.
- Vents unit will require suction and how to complete tube feedings.
- Wound care will also be done.
Experience
- Active BLS required
- Active WI RN license
Pay: $2,273.00 per week
