Job Description – Mentor

The following describes the Services to be performed, any Materials that are to be delivered, and the schedule for delivery (if applicable):

Working hours – As determined by client, Clinical Director and Mentor.

Current hourly rate: $22-$25 per hour

Weekend availability – Variable

Face-to-Face contact with Client is expected.

Mentor Role

• To provide guidance and supervision, and to act as a positive adult role model to the client.
• To understand and administer client’s written treatment plan as required and to work with client first to resolve any issues. The plan was authored by the treatment team in conjunction with the client and specifies a good faith commitment to abide by the established rules and guidelines.
• To assist client with accountability, organization of school and treatment related assignments and materials, calendar planning, time management, home management skills, community service projects, and overall implementation of a full-time program.
• To monitor the client in all his/her activities both in and outside of the home and school.
• To provide transportation for the client when needed.
• To chaperone the client to outside social events, activities, family vacations, or other functions as deemed necessary.
• To assist family consultant in delivering didactic and experiential education sessions that teach daily living skills, social skills, cognitive restructuring, building self-confidence, motivational skills, alternative coping skills, and other skills as needed.
• To work in collaboration with biological parents, clinical staff and family consultant in all aspects of duties and responsibilities.
• Take the lead in supporting a young person in an ongoing, one-to-one relationship.
• Serve as a positive role model and friend.
• Build the relationship by planning and participating in activities together.
• Strive for mutual respect.
• Build self-esteem and motivation.
• Help set goals and work toward accomplishing them.

• Excellent organizational skills, familiarity with systems to assist individuals with community resources/special needs a plus.
• Be health conscious, athletic and in good shape.
• Knowledge of and understanding of the federal confidentiality requirements.
• Ability to provide transportation to the client.

Time Commitment

• Make a minimum of a one-year commitment/ninety days probation.
• Spend constructive one-to-one time with a mentee daily.
• Communicate with the mentee daily.
• Document in writing, time spent with client each day. (Best Notes)
• Attend an initial orientation and training session and any/all additional hands-on training/supervision sessions monthly during participation in the program.
• Attend any additional mentor/mentee group events, mentor support groups, and program recognition events.

Program Activity

• PRN for Families, Inc., acknowledges that participation in any activity inside or outside of the program will involve the risk of injury. Additionally, the employee and/or client understand that their participation in any activity is strictly voluntary and personal. They are aware that participation is not a requirement of any of the duties, responsibilities, or treatment as an employee or client of PRN for Families, Inc. The employee and/or client will attest that they are in good physical condition before participating in any high-risk activity.
• High risk activities would include but not be limited to – white water rafting, off-roading, BASE jumping, hang gliding, race car driving, horseback riding, bungee jumping, flying a plane, and parasailing.
• Any activity must be accepted and approved by PRN’s Program Director prior to the activity and only after all appropriate waiver signatures have been obtained. Copies of the signed activity waiver and emergency forms for each participant (client or employee) will accompany the person that heads up the activity in case of emergency.

Participation Requirements

• High school diploma with equivalent experience.
• Be at least 21 years old.
• Be interested in working with young people.
• Be willing to adhere to all program policies and procedures.
• Be willing to complete the application and screening process.
• Be dependable and consistent in meeting the time commitments.
• Attend mentor-training sessions as prescribed.
• Be willing to communicate regularly with family consultant, submit activity information, and take constructive feedback regarding mentoring activities.
• Have access to an automobile, auto insurance, and a clean driving record.
• Have a clean criminal history.
• No illicit drug uses.
• No alcohol use or controlled substances in an inappropriate manner.
• No history of treatment for substance abuse.
• Pass a comprehensive drug/alcohol screening if requested.
• No history of treatment for mental disorder or hospitalizations for such in the past three years.
• A current medical examination and vaccinations if requested.
• All employee and/or independent contractors whose work includes face to face meetings with clients are requested to record their actual work and hours on PRN’s Electronic System – Best Notes – within 24 hours of each meeting. PRN to provide training on the Best Notes program. Monitoring of the process is completed weekly for compliance.

Application and Screening Process

• Written application
• Motor vehicle record check.
• Criminal history check: state, child abuse and neglect registry, and sexual offender registry.
• Personal interview.
• Provide two personal references.
• Attend monthly mentor trainings when available.
• Stay current and up to date on all required paperwork – certifications, licenses, First Aid, CPR, Medical, Driver’s License, Insurances, registrations, program calls, trainings, etc.

Pay: $22.00 - $25.00 per hour
