In this role you will work within a high-quality, highly communicative/collaborative, and team-based architecture to oversee the comprehensive health and well-being of patients. Physicians will participate in regular huddles with their team to proactively identify health care issues and orchestrate a plan of care with the assistance of their team and the support of an entire organization.

- A group of 25 phenomenal colleagues (Internal Medicine, Pediatrics and Family Medicine physicians) situated in San Francisco, Menlo Park, Los Angeles, New York, and Miami who communicate and collaborate regularly
- Establish and maintain long-term relationships with patients
- Manage chronic and acute illness and injuries
- Provide well checks and house calls
- Be available after-hours by phone for patient communication and care
- Track and review normal physical development as well as psychological well-being
- Establish an outstanding relationships with and lead a team comprised of a Registered Nurse and Patient Care Administrator
- Create personalized reports for patients to help them and the team navigate their health goals and care
- Education level: Graduate of four-year accredited medical school
- Medical Training: Complete residency in Internal Medicine, Family Medicine, or Medicine-Pediatrics program
- Experience: Minimum of five (5) years as a licensed M.D.
- Preference: Work experience includes longitudinal primary care. Have trained or worked in an academic medical center
- Specific skills:
- Outstanding written and verbal communication
- Advanced computer literacy, primarily with iOS
- Experience with EHR-based medical record keeping
- Ability to work effectively with and through teams
- Proven track record of providing extraordinary patient care and customer service
- Enthusiasm in working in collaborative teams
- Excellent ability to multitask and work effectively under pressure
- High emotional intelligence and ability to create and maintain relationships
- Intellectual humility and the ability to communicate compassionately
- Licensed MD in the state of California
- Current BLS certification
