Private Home Care is seeking a compassionate and organized Licensed Practical Nurse (LPN) to join our team in a full-time role supporting one of our valued partner facilities. This position is ideal for an LPN who enjoys building relationships, leading caregivers, and making a meaningful impact on both clients and staff.
Responsibilities:- Oversee and support Private Home Care caregivers within the partner facility
- Create and manage caregiver schedules
- Build and foster strong client and family relationships
- Complete new client assessments and reassessments
- Develop and update individualized care plans
- Collaborate with facility staff and the PHC team to ensure exceptional care and communication
- Provide ongoing guidance and support to caregivers to maintain quality standards
- Other duties as assigned
- Current LPN license in good standing
- 3 years of experience in professional setting
- Strong communication and organizational skills
- Experience in home care, senior care, or long-term care preferred
- Leadership experience is a plus
- Passion for providing compassionate, client-centered care
