Job Summary

Essential Functions

- Performs medical case management functions for patients/clients of the Ryan White Program. Provides treatment adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments.
- Responsible for intake and assessment of service needs.
- Develops a comprehensive, individualized service/action plan.
- Coordinates services required to implement the plan.
- Responsible for client monitoring to assess the efficacy of the plan.
- Responsible for benefit assessments and enrollment, e.g. Medicaid, Medicare, ACA, and ADAP.
- Provides coverage in the duty room to assist clients with Patient Assistance program applications.
- Serves as a member of the interdisciplinary health team.
- Performs other duties as assigned.
- This is a non-management job that will report to a supervisor, manager, director or executive.
- Education - Bachelor’s degree from an accredited School of Social Work or related field of study. Licensed Bachelor of Social Work (LBSW) preferred.
