## Key Responsibilities### Assessment & Planning
- Conduct comprehensive psychosocial and behavioral assessments in home and community settings
- Identify strengths, needs, triggers, and barriers impacting independent living
- Develop individualized person-centered service plans aligned with PBIS principles and ILST goals### PBIS / Behavioral Support Services
- Implement Positive Behavioral Interventions and Supports (PBIS) strategies tailored to each individual
- Identify behavioral patterns and develop proactive, preventive interventions
- Support skill-building to reduce challenging behaviors and promote adaptive functioning
- Collaborate with individuals and care teams to reinforce positive behavioral outcomes###
- Collaborate with service coordinators, healthcare providers, therapists, and family members
- Participate in interdisciplinary team meetings and service planning reviews### Documentation & Compliance
- Maintain timely, accurate documentation in compliance with NYS Medicaid waiver requirements
- Track client progress toward behavioral and independent living goals
- Ensure services meet NHTD/TBI program standards and guidelines
- Bachelors degree in Social Work, Psychology, or related human services field (MSW preferred)
- Experience working with individuals with TBI, developmental disabilities, or behavioral challenges
- Strong understanding of Positive Behavioral Interventions and Supports (PBIS)
- Familiarity with NHTD and/or TBI waiver programs (preferred but not required)
- Excellent communication skills
- Strong documentation and organizational skills
- Ability to work independently in community-based settings
- Valid drivers license and reliable transportation required (preferred but not required)
