Job Summary
Priority Care is seeking a compassionate and reliable Family Support Specialist to provide direct support to individuals with developmental disabilities and their families in home and community-based settings. This role focuses on promoting independence, supporting daily living skills, and enhancing quality of life through person-centered services.
Key Responsibilities
- Provide direct support in the home and community (ADLs, routines, skill-building)
- Assist individuals with community integration (appointments, outings, activities)
- Support behavior plans and individual goals as outlined in ISP/PCP
- Maintain accurate and timely documentation (daily notes, progress updates)
- Communicate effectively with families, guardians, and team members
- Ensure the health, safety, and well-being of individuals at all times
- Transport individuals as needed (must have reliable vehicle)
Qualifications
- High school diploma or GED required
- Experience in DD/IDD, behavioral health, or caregiving preferred
- Must be at least 19 years old (Nebraska requirement)
- Valid driver’s license, reliable transportation, and auto insurance
- Ability to pass background checks (APS/CPS, criminal history)
- CPR/First Aid (or willingness to obtain)
