Priority Care Alliance is seeking a professional, outgoing Community Relations & Outreach Specialist to represent our home care and home health services throughout the community. This role focuses on in-person relationship building, community engagement, and referral development with healthcare and community partners.
This position begins as a PRN contract role with the opportunity to grow into a full-time position based on performance and organizational needs.

Key Responsibilities:
• Build and maintain relationships with:
• Physicians’ offices
• Hospitals, rehab centers, and skilled nursing facilities
• Assisted living and personal care homes
• Community organizations and senior-focused programs
• Serve as a professional brand ambassador at:
• Healthcare facilities, partner locations, and community settings
• Health fairs, expos, and networking events
• Participate in engagement activities (e.g., bingo, wellness events, informational sessions) at partner facilities
• Represent Priority Care Alliance services accurately using approved materials
• Track outreach efforts and referral interactions
• Collaborate with leadership to support referral growth and brand presence
Qualifications:
• Strong interpersonal and communication skills
• Comfortable with in-person outreach and public engagement
• Professional appearance appropriate for healthcare environments
• Experience in healthcare, home care, sales, marketing, or community outreach preferred
• Reliable transportation required
• Knowledge of home care or healthcare referrals a plus (training provided)
