We are growing and seeking a Hospice Registered Nurse (RN) Case Manager to join our team and provide skilled, compassionate care to patients and families across the Portland metro area. If you’re a nurse who values autonomy, meaningful relationships, and whole-person care — we’d love to meet you.

⭐ What You’ll Do

• Provide skilled nursing care to hospice patients in homes and facilities
• Complete comprehensive assessments and develop individualized Plans of Care
• Manage pain and symptoms with a holistic, patient-centered approach
• Educate patients and families about the disease process, medications, and comfort measures
• Serve as the primary case manager and coordinate care with the interdisciplinary team
• Communicate changes in condition to the attending physician and IDG
• Supervise CNAs and LPNs involved in the patient’s care
• Document timely and accurately in the EMR system
• Participate in IDG meetings, care conferences, and quality initiatives
• Rotate after-hours / on-call responsibilities as assigned

⭐ What We’re Looking For

• Current Oregon Registered Nurse (RN) license
• One year of clinical nursing experience (hospice or home health preferred)
• Strong assessment, communication, and critical-thinking skills
• Ability to work independently and manage a caseload effectively
• Compassion for patients and families during end-of-life care
• Valid driver’s license, reliable vehicle, and auto insurance
• BSN preferred; MSN desired

Pay: $40.00 - $45.00 per hour
