Position Summary
The HOPE Coordinator serves as the operational lead for the Hospice Outcomes & Patient Evaluation (HOPE) assessment lifecycle, ensuring accurate and timely data collection, validation, and submission in compliance with CMS HQRP requirements. The role ensures that HOPE-ADM, HUV1, HUV2, SFV, and HOPE-DISCH are completed accurately, integrated into the EHR, and submitted through iQIES within CMS-mandated timeframes. The HOPE Coordinator also supports clinical teams with education, documentation oversight, and QAPI initiatives to optimize quality outcomes, compliance, and survey readiness.
Essential Duties & Responsibilities
1) Regulatory Ownership & Calendar Control

Maintain the enterprise HOPE calendar and enforce CMS time windows:
- HOPE-Admission during/with the comprehensive assessment 5 calendar days after election.
- HUV1 on/ between days 6–15 after election.
- HUV2 on/ between days 16–30 after election.
- SFV (Symptom Follow-up Visit) within 2 calendar days when moderate/severe pain or non-pain symptom impact is documented at Admission/HUV.
- HOPE-Admission during/with the comprehensive assessment 5 calendar days after election.
- Track exceptions and document when a HUV is not required/accepted per CMS scenarios.
- Own go-live/ongoing submission operations in iQIES.
2) Documentation Governance (EHR Build & Clinical Content)

Map every HOPE item to EHR forms, flowsheets, and care-plan fields; ensure single-entry, multi-use data capture.
- Embed communication needs in Admission in line with PC.C35 and HOPE items.
- Ensure plan-of-care updates are tied to HUV findings; reinforce 15-day reassessment cycles and IDG reviews.
3) Submission, Corrections & Data Quality (iQIES)

Oversee file creation, validation, and submission of all HOPE records in iQIES; monitor acceptance reports, edit/out-of-sequence warnings and resolve quickly.
- Administer the HOPE Record Correction Policy: initiate and track corrections for any accepted record, ensuring the EHR and iQIES match.
4) Training, Competency & Field Support

Design and deliver role-specific trainings covering timepoints, SFV triggers, item coding, and documentation standards.
- Clarify who may perform each timepoint per scope and agency policy; reinforce that SFVs can be completed by an RN or LPN/LVN when clinically appropriate.
- Maintain quick-reference guides, case scenarios, and an escalation “hotline” for day-of questions.
5) Performance Analytics & QAPI

Build dashboards for timeliness completeness, acceptance, corrections, and measure rates; present monthly to QAPI and Governing Body.
- Monitor HOPE-based HQRP measures and drive action plans with service line leaders.
- Lead targeted audits across active and discharged records; integrate findings into PI plans and education.
6) Policy Alignment & Survey Readiness

Keep Primecare policies current with HOPE releases, CMS FAQs, and iQIES guidance; publish update memos and revise SOPs.
- Maintain a survey binder: policies, training rosters, audit results, iQIES acceptance reports, correction logs, competencies, and crosswalks too.
- Serve as point-of-contact for CMS/state surveyors regarding HOPE submissions and clinical documentation access.
7) Patient/Family Communication & Equity
- Ensure documentation reflects patient preferences and effective communication at Admission and throughout updates, consistent with Patient Rights and; audit that interpreter usage is recorded when applicable.
Qualifications

Licensed Registered Nurse in the State of Georgia, clinically experienced professional with 2–3 years hospice documentation/quality experience; deep knowledge of comprehensive assessment and IDG process.
- Demonstrated experience with EHR build/optimization and quality reporting.
- Strong analytics and project management; advanced Excel or BI tools.
