RN Case Manager | Cape Cod Hospital, Massachusetts

Cape Cod Hospital is seeking an RN Case Manager to coordinate patient care, utilization review and discharge planning. The role works with physicians, patients, families and payers to support safe transitions of care and appropriate coverage.

Key responsibilities

• Screen new admissions for discharge planning needs and complete initial assessments in InterQual
• Review admission status, level of care and continued stay using InterQual criteria
• Participate in daily rounds and coordinate discharge plans, referrals and transfers
• Provide clinical information to payers, address potential denials and help patients understand coverage decisions and appeal rights

Requirements

• Active Massachusetts RN registration
• At least 3 years of acute care experience within the past 5 years, with broad hospital or case management experience
• Recent discharge planning and utilization review experience within the past 4 years
• Working knowledge of InterQual or an equivalent system; acute care and InterQual are specifically listed as must-have skills

A BSN and case management, CPUM or specialty certification are preferred. The posting does not specify a shift, contract length or guaranteed hours.
